Annual conference programs and preconference.

FORM A
Combined YALSA Form A and ALA Call for Proposals for Conference Programs
Preliminary Proposal for Programs and Preconferences
Due by the Monday morning of Midwinter 18 months before the Program
Form indicates Tracks, Day, Time, and Program Description

FORM B
Final Proposal for Programs and Preconferences
Detail of Speakers, Format, Equipment
Due 12 months before the Program,
by the Monday afternoon of the preceding Annual Conference

FORM C
Program Co-Sponsorship Proposal
Due 12 Months Prior to the Program

FORM D
Program Evaluation
To be distributed to audience members at the program, picked up and collated by the Program Chair and sent to the Program Clearinghouse Committee Chair by the Program Chair

FORM E
Program Evaluation Summary
Must be sent by the Program Chair to the Program Clearinghouse Committee Chair and the YALSA Office
by August 1 following the Program
