ALSC at the 2010 ALA Annual Conference

Program Organizer Planning Forms

Complete Program Planning Guide (MS Word)- This is a read-only copy of the planning guide to print out.  Unfortunately, you will not be able to electronically complete and save individual forms.  Please use the individual forms that appear below.  You may download the form, enter the appropriate information, save, email,snail mail, or fax back to the ALSC office.

Individual Planning Forms

  • Program Organizer Form (MS Word)- To be submitted by the primary person the ALSC office will correspond throughout the year.  Only one person per program.  Due Sept. 11, 2009
  • Program Description Form (MS Word)- Use for both your preliminary description to use during Midwinter and to submit your final program description for the onsite Program Guide.  Due Oct. 2, 2009
  • Room Set-up Form (MS Word)- Indicate if you would prefer theater or round tables, ADA requirements, or other special set-up needs. Initial form due Oct. 2, 2009; final form due Feb. 5, 2010
  • Audiovisual Equipment Request Form (MS Word)- Check with all of your presenter's for their audiovisual needs and submit on one form.  Initial form due Oct. 2, 2009; Final form due Feb. 19, 2010
  • Speaker Contact Form (MS Word)- As you confirm speakers, send in a completed form for each person so that they will appear in the program guide and receive a confirmation letter.  Initial due Oct. 2, 2009; final form due Jan. 25, 2010
  • Handout Duplication Form (MS Word)- Attach with each presenter's handout masters so that the ALSC office can duplicate handouts and ship to the conference.  Also remind speakers to upload their handouts (preferablly prior to conference) on the ALA Conference Presentations Wiki (http://presentations.ala.org).  Due May 14, 2010
  • Evaluation Form (MS Word)- Please fill out an evaluation form after your program is completed.  Your feedback is important to ensure successful programs in the future.  Due July 9, 2010
